We are a Not-For-Profit agency in Jefferson County that provides employment supports to individuals with disabilities.
The Supported Employment Case Manager will have a caseload of individuals (clients) in various phases of programing.
The Supported Case Manager will carry out exploration activities with clients to determine work readiness and to develop job goals and recommendations.
The Supported Employment Case Manager will also oversee and work with team as clients receive supports to obtain employment, receive job coaching supports on the job, and long-term job retention supports.
PAY BASED ON EXPERIENCE STARTING AT $21.

Community based Job Discovery and Exploration activities with clients.
Oversee, support, and provide documentation for clients as they are in job placement, job coaching, and follow along programming.
Communication with support teams:
Vocational Rehabilitation counselor, employers, families, teachers, etc.
Qualifications and Skills Bachelor's degree in Social Services or related field preferred.
valid driver's license with good driving record.
Will use agency vehicle to transport clients for job exploration activities.
reliable to work independently without direct supervision, and work well with a team.
ability to communicate in an effective and clear manner, both oral and written form, including computer and typing skills for documentation.
Organization and time management skills.
